ESWT for Winter Athletes: The Game-Changer for Stubborn Tendon Pain

As Anchorage transitions into ski, skate, and snow-running season, we see the same pattern every year: the snow arrives, athletes ramp up, and old tendon pain suddenly becomes the reason winter adventures get cut short.

Achilles irritation from fall running. Patellar tendon pain that lights up on climbs. Elbow or forearm pain from early-season climbing. Plantar fascia flare-ups after the first snowy run.

If this sounds familiar, you’re not alone—and you don’t have to sit this season out.

At Runners’ Edge Alaska, Extracorporeal Shockwave Therapy (ESWT) is one of our most effective tools for active Alaskans battling persistent tendon pain.

What Is ESWT?

ESWT is a non-invasive therapy that delivers high-energy sound or acoustic waves to treat injured tendons and soft tissues.

This targeted energy stimulates healing in ways traditional physical therapy approaches simply can’t (at least not as quickly or efficiently!).

Here’s what ESWT does inside the body:

  •  Desensitizes painful tissue
  • Increases blood flow
  • Stimulates stem cell activity
  • Boosts collagen production
  • Breaks dysfunctional scar tissue
  • Promotes tissue remodeling
  • Supports long-term tendon resilience

It’s one of the reasons ESWT has surged in popularity over just the past few years—it works.

Two Types of ESWT at Runners’ Edge Alaska

We use both Radial and Focused ESWT to tailor treatment to your specific injury.

1. Radial ESWT (R-ESWT)

Best for:
• Plantar fasciitis
• Achilles tendon pain
• Patellar tendinopathy
• Medial/lateral elbow issues
• Broader, surface-level pain

Radial waves disperse outward to treat larger, more superficial areas—perfect for tendon irritation near the surface.

2. Focused ESWT (F-ESWT)

Best for:

  • Deep tendon pain
  • Chronic tendon injuries
  • Bone stress injuries
  • Insertional Achilles problems
  • Deep hip/gluteal tendinopathy
  • Cases that haven’t responded to conservative care

Focused ESWT penetrates deeper and more precisely, making it the tool of choice for stubborn or long-standing tendon issues.

Your PT will evaluate your injury and decide which type—or combination—will get you back to your sport fastest.

Common Tendon Issues We Treat — Especially in Winter Athletes & Skiers

Cross-country skiers and winter athletes frequently develop tendinopathies due to repetitive pole use, power transfer demands, and repetitive loading on snow.

Here are the most common:

  1. Rotator Cuff Tendinopathy: Crucial for pole planting and upper-body propulsion.
  2. Lateral Elbow Epicondylitis (Tennis Elbow): Repetitive pole use = irritation at the elbow.
  3. Patellar Tendinopathy (Jumper’s Knee): Classic in skiing due to the heavy knee flexion and force absorption.
  4. Achilles Tendinopathy: Vital for push-off, climbing, and power transfer on snow.
  5. Plantar Fasciitis: Often flares when winter runners transition to ice, snow, and uneven terrain.

What Does a Typical ESWT Protocol Look Like?

Your PT will customize your plan, but what the research has found most effective is:

  • 3–6 total sessions- Often spaced 1 week apart.
  • Improvements start between weeks 3–6 – Although you may feel temporary relief immediately after treatment.
  • The 8–12 week window is where major remodeling happens- Collagen reorganizes. Tendon capacity increases. Risk of re-injury declines.
  • Always paired with a tendon loading program- This might include:
    • Progressive strength training
    • Mobility work
    • Return-to-sport progression
    • Skiing or running technique modification
    • Education on load management

The combination of ESWT + targeted rehab is where the magic really happens.

Is ESWT Safe?

Yes. ESWT is generally very well tolerated.

Most people experience only temporary redness or mild soreness at the treatment site (this means it’s working!). Except for cases of acute injury, there’s typically no downtime and no restrictions.

Receiving treatment from a trained sports PT ensures the correct dosage, technique, and follow-up rehab for the best results… and to make sure the pain doesn’t come back!
